Submitting Unanticipated Problems using OSIRIS
The IRB is happy to announce that unanticipated problem reports (i.e., adverse events and other unanticipated problem reports) can now be submitted using OSIRIS. As with all other submissions in OSIRIS, the application is built using smartform questions and uploading regulatory documents. Please note that this process is available for electronically approved studies only. In order to initiate this process, click on the "new unanticipated problem" link available from the study workspace under "My Activities." Studies currently approved in the paper format will continue to have all correspondence processed in paper.
